If your property is in a city for which a special form is not included in the folder, you will use the "General" notice.

Under the "Resources" folder in the same link, we have included pre-eviction filing checklists to help you understand when you may submit your nonpayment case for filing after October 1, for both Transition Period notices (where 25% has not been paid) and Recovery Period. We have also included a letter template to give to a tenant if you voluntarily choose to remind them of the amount they must pay to meet the 25% payment requirement.
